I, being the representative of the group registering for this event, do understand that it is my responsibility to collect and store the liability & medical release forms, provided online or in person by Ablaze Ministries, for the participants registering with my group.

The collected liability & medical release forms will be brought with me to the event and accessible to me at any time during the event, using either physical or electronic means, in case of an emergency involving the participants within my group.

To the extent permitted by law, I release and agree to indemnify and hold harmless Ablaze Ministries and the Catholic Diocese of Austin, including their employees, contractors, clergy, agents, facilitators and volunteers from any and all liability, claims, demand and costs which may arise as a result of this group's participation in this event or which is, in any way, related to such participation. This paragraph covers loss under any theory of loss (negligence or otherwise) including but not limited to personal injury or property damage. The participants and their parents/guardians assume all risk of injury or loss to themselves or their property.
